Thank you for taking part in this important health survey. Your responses are completely anonymous and invaluable to the future of our breed.

Please complete the survey for each Portuguese Water Dog (PWD) you currently own—or have owned—that was born in 2005 or later.

Your participation will help us better understand the current health status of PWDs and identify any emerging concerns within the breed that we can share with researchers and veterinarians. The insights gathered will guide efforts to protect and improve the health of our dogs for generations to come.

The survey should take less than 10 minutes per dog. We appreciate your time and commitment to the wellbeing of Portuguese Water Dogs.
